Preparing for major surgery typically involves fasting, medication adjustments, and logistical planning. But a growing body of evidence suggests that the mind deserves as much attention as the body. A new study, published in a leading European medical journal, argues that psychological preparation is a critical—and often overlooked—component of surgical success.
The research, conducted by a team at the Karolinska Institute in Stockholm, followed more than 1,200 patients undergoing elective procedures in Sweden, Germany, and the Netherlands. It found that those who participated in structured mental preparation—such as guided imagery, relaxation techniques, or cognitive behavioural therapy—reported significantly lower post-operative pain, shorter hospital stays, and a faster return to daily activities.
Why the mind matters on the operating table
“Surgery is not just a physical event; it is a psychological one,” said Dr. Elin Bergström, the study’s lead author. “The body’s stress response, which is heavily influenced by mental state, can either impede or accelerate healing. When patients are anxious or fearful, their bodies produce higher levels of cortisol, which can suppress the immune system and slow tissue repair.”
The findings align with a broader shift in European healthcare towards holistic, patient-centred approaches. In the United Kingdom, for example, the National Health Service has begun piloting pre-surgical “wellbeing clinics” in Manchester and Leeds, where patients learn breathing exercises and mindfulness techniques. Meanwhile, in Germany, several university hospitals—including Charité in Berlin—now offer psychological screening as part of the standard pre-operative assessment.
“We’ve long known that a positive mindset can influence outcomes, but this study gives us concrete data,” said Professor Markus Weber, a surgeon at the University of Zurich who was not involved in the research. “It’s time to treat mental preparation as seriously as we treat physical preparation.”
The study also highlights the role of technology. In a separate development, AI-guided brain surgery in the UK is already helping to preserve patients’ sight during delicate operations. Such advances, combined with psychological support, could redefine what it means to be “ready” for surgery.
However, the researchers caution that mental preparation is not a one-size-fits-all solution. “Some patients respond well to mindfulness, others to cognitive reframing, and others simply need more information about what to expect,” said Dr. Bergström. “The key is to personalise the approach.”
The study’s authors are now calling for a Europe-wide protocol that would integrate psychological assessment into pre-surgical care. They point to the Basque screening programme, which has dramatically reduced colon cancer deaths, as an example of how systematic, preventive approaches can transform outcomes.
For patients, the message is simple: don’t neglect your mind when preparing for the operating theatre. As the study suggests, a calm, informed, and mentally resilient patient is more likely to wake up on the road to recovery.
